Tips before you share
- Ask for the rating first — it is effortless and gets people started.
- Keep open questions to two or three; response quality drops after that.
- Make identity optional if you want candid answers.
- Ask about one experience (an event, a visit, an order), not “everything”.

Should feedback forms be anonymous?
Anonymous forms tend to get more candid criticism; named forms let you follow up. A good compromise is an optional name and email field, which this template uses.
What questions should a feedback form include?
An overall rating, what went well, what should change, and whether they’d come back or recommend you. More than that and completion rates fall.
